Obtaining Birth and Death Certificates
Certified copies of most Missouri birth and death records may be obtained through our office. We can access birth records from 1920 to present and death records from 1980 to present.
- Completed Application Form
- Payment by cash, check or credit/debit card.
- One primary form of identification that includes your name and photo.
- (Examples: a valid Driver’s License, U.S. Military ID Card, U.S. Passport, Work or School ID…)
OR
At least two alternate forms of identification (Documents must contain your name and the name of the company or organization issuing the document.)

Birth and Death Certificates may be obtained in 3 Ways
Walk-in
Same day service is available to walk-in customers at our North and South Office. This is the fastest way to obtain a birth/death certificate.
North Office Hours:
Monday – Wednesday:
8:00 am -4:30 pm
South Office Hours:
Monday – Thursday:
7:00 am – 5:30 pm
Friday: 7:00 am – 12:00 pm
Drive-Thru access is available at the South Office during regular business hours.
You may download and complete the application. Application must be notarized by a licensed Notary Public. Upon completion it may be sent to our office (along with a self-addressed stamped envelope, photo copy of your driver’s license, and application fee). Certificates will be processed and returned to sender within two to six weeks. Information may be mailed to:
